We welcome new patients and will be happy to include you on our list. However, if you do not provide the information that we require, we have the right to refuse your application.

If you wish to register as a new patient, please request for a registration form from reception. All new patients are required to provide us with their NHS number, proof of address and proof of identity. You may also be requried to provide us with proof that you are eligible for treatment on the National Health Service.

If you reside within the surgery's allocated area and you have provided us with all the information we have requested, we will endeavour to register you within two weeks of your submitting your completed application. We will confirm this in writing and invite you to attend a new patient health check with our pratice nurse. We would encourage all new patients to take this up as it would give us an opportiuniy to record a summary of your medical history and ensure that if you are on any medication this is entered into your records without any undue delay. Young children's immunisation status could also be updated at this point.

Please note that once you have applied to be a patient on our list, you are entitled to be seen by a doctor or the nurse as a temporary patient until your registration is complete.

Change of Contact Details

Patients are reminded that it is extremely important to notify the practice of any changes to their contact details, be it a change of address or change of telephone number. This is very important in order for us to contact patients if the need arises. This may be in regard to their health and wellbeing or even for instances when there may be a pandemic and they are required to be immunised.

Please inform Reception as soon as possible either by telephone or in writing to advise any changes as appropriate.

Please note that if you your new address is outside of our catchment area you may need to register with another GP practice who is closer to your new address.

Temporary Residence

We understand that sometimes patients have relatives and/or friends visiting them for a few days. During this time those people may fall sick and may be in requirement of medical attention. We will be happy to see those patients on a temporary basis. Please contact reception for further details.
